Good health requires timely thorough medical information. We are encouraging our patients to complete an online medical questionnaire prior to their visit and the results are completely confidential and only available to you. You will need to print out the results and bring them with you so this requires that your computer is hooked up to a printer. This is optional and is meant to help you. Many people forget to report some of their symptoms and problems at a visit and the information in this will help us to better treat you.

Our Practice
